Schedule of Medications, Etc.  When completing any paperwork for your case, be sure to write your name, soc sec number and the current date on the top of each page, and number the pages.


An Excel spreadsheet with grid lines printed makes a nice blank schedule.  Because of technical difficulties, it was not possible at this time to provide a reproducible blank schedule.  However, a blank schedule without the headings/lettering can be obtained by clicking here.

          SCHEDULE OF MEDS  -  PHOTOGRAPHS
PHOTOGRAPHS ARE WORTH A THOUSAND WORDS.

Part of ones documentation can be photographs of their MCS/EI/CI accomodations.  This can tell a story about our special needs better and quicker than one can imagine.  Take the photos to your doctors and lawyers.  Give them copies.  Following are a couple of samples:
     AN MCS BED

This is a bed made of bare bedsprings with a very slim layer of tolerated fabric for cushioning.  This lady is allergic to cotton so she cannot even tolerate the better cotton mattresses.

When she cannot sleep on this "bed" she is on the floor with her bedding.
"My doctor regularly flips to this picture in my file and comments on the severity of my condition to his nurse."
  AN MCS COMPUTER

This is a computer with a homemade ventilation system to vent the fumes out the window.  There is also an air purifier in the room that is used as needed.  In use here is an older monitor with poor resolution.  A newer monitor had to be returned because of adverse reactions.
"Inspite of the venting system, I have to limit my time on the computer.  It zaps my energy and my brain dysfunctions after not very long."
